Integrating Traditional Medicine into a modern health care system

Abstract

Traditional Medicine collectively referred to as complementary and alternative medicine (CAM) when commonly used outside their traditional context, alongside other medical systems, including Western biomedicine.The World Health Organization officially promoted traditional medicine in developing countries in 1978, there have been increasing interests among developing countries in integrating traditional medicine into a national health care system. Integrating traditional medicine into a modern health care system, moreover, can benefit industrialized nations as well. The contributions of Traditional and Modern Scientific Medicines to health care delivery have attracted a great deal attention in most communities worldwide. Traditional Chinese medicine, Ayurveda, Kampo, traditional Korean medicine, and Unani have been practiced in some areas of the world and have blossomed into orderly-regulated systems of medicine. More than 80% of the world’s population in over 170 of WHO’s 194 Member States currently use some form of traditional medicine, such as herbal medicine, yoga, Ayurveda, acupuncture and acupressure, and indigenous therapies.To generate awareness about traditional medicine, since the 1980s, a number of publications on self-health care have been developed to inform people about the benefits and uses of traditional medicine. Some of the areas of focused research include studies on the development of anticancer drugs, cardiovascular diseases such as arteriosclerosis and angina pectoris, respiratory diseases such as bronchial asthma, obesity, diabetes and other metabolic disorders, and basic studies on acupuncture therapeutic mechanisms for various bone and joint and spinal disorders, and on different kinds of composition of the human body. To ensure the safety, standardization, efficacy and quality of traditional medicines, the practitioners must follow the same stringent standards and regulations for production and use of traditional medicines as are followed for allopathic medicines. This study aims to summarize the advancements made in understanding the efficacy, effectiveness of Traditional Medicine. Traditional and local knowledge systems need to be protected, preserved, and studied as different ways to approach modern healthcare, science, and technology at large. Significant challenges exist in integrating the differing perspectives. Traditional knowledge is derived from years of history and experience and is preserved through long, complex narrations lacking the traditionally rigorous scientific scrutiny required by modern medicine. Modern scientists are prone to quickly dismiss its merit, considering it to be irrelevant as a result. For many, traditional medicine is the first port of call, and practitioners of traditional medicine have played an important role in treating chronic illnesses. These traditional medicines and practices have been preserved, organized and modernized during the past several decades, and have been fully integrated into the national health-care delivery systems from the central to the most peripheral administrative levels.

Acupuncture & Electrotherapy: an alternative and complementary treatment method

Abstract

 Acupuncture involves activating specific points on the skin, usually by inserting needles.  Acupuncture was based on the principles of traditional Chinese Medicine.  Traditional acupuncturists understood health in terms of a vital force or energy called "Qi" that circulates between the organs along channels called meridians. The flow of 'Qi' energy must be of the right strength and quality in each of these meridians and organs so that health is maintained.  Acupoints are located along the meridians and can alter the flow of energy and appear to correspond to nerve endings. There is a distinct difference between traditional and Western acupuncture, but the two approaches overlap significantly. In terms of Western theories, acupuncture induces signals in central nerves that modify the transmission through the spinal cord, as well as the perception of pain in the brain. Electrotherapy leads to stimulation of the sensory and motor nerves, and is especially used in the treatment of pain. Different set of parameters may lead to different physiological effects and treatment may be more specific. Electroacupuncture can be performed with or without needles, activating traditional acupuncture points or choosing points that have a neural background, such as trigger points or tender points. The purpose of this study is to verify that acupuncture works as an effective alternative and complementary treatment method. It is a safe, effective and natural approach, which helps to regain and maintain health and mental well-being.

Abstract

Word "pain" derives its origin from the Indo-European root aleg meaning to suffer. Word "pain" is later and comes from the Latin word "poena" meaning punishment. Since ancient times there has been disagreement regarding the perception of pain and its evaluation. Unlike sight, hearing and smell, pain does not seem to be a primary sensation, but rather an emotional experience. most pain researchers view pain as a complex perception, induced by noxious stimuli. Although pain is the most frequent symptom in medicine and despite the enormous advances that have been made in the field of analgesia and anesthesia, the pathophysiological mechanisms involved in its generation and maintenance are not fully understood. Definition of pain was given in 1979 by the classification committee of the international association for the study of pain (iasp) "as an unpleasant aesthetic and emotional experience, associated with actual or potential tissue damage or described in terms of such damage". In other words, although physiology and anatomy determine a precise point of reference for the detection and transmission of messages interpreted as painful, what differentiates the experience of pain is the fact that there is always an emotional gradient to the experience of pain.

Pain control & acupuncture

Abstract: 

Acupuncture is used mainly to relieve discomfort associated with a variety of diseases and pain conditions Over the years there has been substantial debate about whether acupuncture really works for chronic pain. Research from an international team of experts adds to the evidence that it does provide real relief from common forms of pain. Despite wide use in clinical practice, acupuncture remains a controversial treatment for chronic pain. Referral for a course of acupuncture treatment is a reasonable option for a patient with chronic pain. The aim of the study was to assess the effectiveness of acupuncture as a treatment of chronic pain, within the context of the methodological quality of the studies. Complementary medicine databases, bibliographies and articles were searched. Our objective was to determine the effect size of acupuncture for chronic pain conditions. We searched MEDLINE and the Cochrane Central Registry of Controlled Trials randomized trial. We included randomized trials of acupuncture needling versus either sham acupuncture or no acupuncture control. We conclude that acupuncture is effective for the treatment of chronic pain, with treatment effects persisting over time.

Acupuncture for Low back Pain & Migraine

Abstract

Acupuncture has been increasingly used as an integrative or complementary therapy for pain. It is well-tolerated with little risk of serious adverse effects. Traditional acupuncture and nontraditional techniques, such as electroacupuncture and dry needling, often result in reported pain improvement. Acupuncture is an ancient Chinese medical practice that involves inserting thin needles into specific points on the body. Acupuncture can be used to treat a wide range of health conditions, including migraines and low back pain. Acupuncture works by stimulating the nervous system, releasing endorphins and increasing blood flow to the affected area. Acupuncture may also help to reduce inflammation and improve circulation in the body. Controlled trials have been published on pain syndromes, such as acupuncture for acute and chronic low back pain, knee osteoarthritis, headache, myofascial pain, neck pain, and fibromyalgia. For some conditions, enough data are available for systematic evaluations or meta-analyses. Acupuncture may provide modest benefits in the treatment of chronic low back pain, tension headache and chronic headache, migraine headache prophylaxis, and myofascial pain.The benefits of acupuncture for people with back pain has been well researched compared to many other conditions, with more higher quality clinical trials. This systematic review will focus on the use of acupuncture and its role in the treatment of low back pain to help better guide physicians in their practice. To determine the effects of a policy of ‘use acupuncture’ on headache, health status and chronic pain. The results of this study will determine if acupuncture, alone or as an adjunct to pharmacotherapy provides effective, safe and acceptable pain relief for patients.

The use of hypnosis as a tool for relieving pain

Abstract

Chronic pain is best understood as a disease process rather than a symptom. Underlying mechanisms for chronic pain are,  nociceptive- tissue damage, neuropathic - sensory, nervous system damage, central heightened pain sensitivity in the central nervous system. Chronic pain has significant cognitive, affective, and interpersonal components. Effective chronic pain management is focused on maximizing function and limiting disability, not just on reducing pain. Non-pharmacological pain management is method utilizes ways to alter thoughts and focus concentration to better manage and reduce pain. Hypnosis is an effective method for acute and chronic pain, but there are also a few limitations.  It is only effective in patients with high hypnotic susceptibility. The anticipated analgesic effect is not achieved in low to moderately hypnotizable patients. The cognitive behavioral approach can be used in patients who do not practice self-hypnosis. There are cases where patients with chronic pain have psychological problems. These patients require psychodynamic therapy, which is not in the range of pain medicine. Hypnosis may be a helpful non drug therapy to reduce pain in chronic conditions.

Abstract: Post-operative pain after inguinal hernia surgery is attributed to surgical manipulation or placement of the preperitoneal mesh. Perioperative use of acupuncture can probably be a useful adjunct for postoperative analgesia. AIM: The aim of this study was to evaluate the effect of EA in mesh inguinal hernia open repair, using pain scales, anxiety questionnaire, the evaluation of pain with an algometer and measurements of stress hormones. Methods, Participants: 54 male patients were included in the study (23 inguinal left and 31 with inguinal right, classification in ASA I-II) submitted in programmed mesh inguinal hernia open repair with the technique Lichtenstein. Investigation parameters included: 1) Pain scales (VAS, PPI, VRS, SS, FS) and the anxiety questionnaire at 30′, 90′, 10 hours and 24 hours postoperatively. 2) Pain threshold and tolerance were evaluated preoperatively, before and after electroacupuncture, and postoperatively at 30′, 90′, 10 hours and 24 hours after surgery. 3) Blood levels of stress hormones cortisol, corticotropin and prolactin were measured at the same time points (excluding 24 hours). The frequency of complications of opiates was recorded. Patients were randomly allocated in 3 treatment groups of 18 patients. The three groups were: Group 1: placebo EA, Group 2: preoperative (40′) and postoperative (60′) EA, Group 3: preoperative, intraoperative and postoperative EA. The trial used low frequency EA of 2 Hz and frequency scanning mode. Needles were placed bilaterally at points of great analgesic effect. Electroacupuncture was applied to the points in pairs SP6–ST36; LI4–PC6; Shen-Men 55-Thalamus 26a. If the pain VAS score was greater than or equal to 3 cm within 90 minutes after surgery, an intravenous bolus dose of 5 mg pethidine was given and continuous intravenous infusion pump of pethidine at a rate of 10 mg/h was administered for 12 hours.

Abstract

Acupuncture therapy has been known as a practice related to oriental medicine, and recently has been detected as a potential therapeutic tool for which there is good scientific evidence. Depression and anxiety are usually classified as mental illnesses and it is more useful to think of them as disturbances in brain health, which is directly related to the physical makeup and brain mechanisms and emotional and relational issues. The ancient Chinese practice of acupuncture could be used for the treatment of depression and anxiety, instead of drugs. The purpose of this review was to summarize the existing evidence on acupuncture as a treatment for anxiety and depression. Search of the literature on acupuncture treatment was limited to specific mental health conditions: depression and anxiety disorder. The literature review focused on systematic reviews. Search focused on the latest versions. The available resources provide some evidence that acupuncture is an effective treatment for these conditions. The findings of the studies show that acupuncture can play an important Role in the treatment of depression and anxiety.
